WhatsApp Business API for Hospitals and Clinics in India: Cost, Setup and Rules
Is the WhatsApp Business API free? How hospitals get it, what it costs, how it differs from the Business app, and the opt-in and template rules.

Quick answer: the WhatsApp Business API (now called the WhatsApp Business Platform) is Meta's official way for a hospital's software to send and receive WhatsApp messages automatically. It is not free: Meta charges per message for business-started messages, and your software provider may add its own fee, but replies to a patient within 24 hours of their last message are treated as service conversations. A hospital cannot just switch it on from the WhatsApp Business app — it needs a verified business, a phone number set up for the platform and a provider or software that is connected to it. For most hospitals, the easiest route is clinic or hospital software that already includes it.
This guide explains what the WhatsApp Business API is, how it differs from the WhatsApp Business app, what it costs, how to get it, and the rules for healthcare messages.
WhatsApp Business app vs WhatsApp Business Platform (API)
| WhatsApp Business app | WhatsApp Business Platform (API) | |
|---|---|---|
| Who uses it | Small businesses on a phone | Businesses with software that sends messages |
| How you send | By hand from the app | Automatically from your system |
| Automatic reminders | No | Yes, with approved templates |
| Chatbots and integrations | Basic auto-replies only | Yes |
| Cost | Free | Per-message charges plus a provider fee |
| Setup | Install the app | Business verification, a number and a provider or direct integration |
| Risk | Low | Low — this is the official route |
A hospital with a few patients a day can run on the Business app. A hospital that wants a confirmation, reminder and token for every patient needs the platform.
What hospitals use the API for
- Appointment confirmations — doctor, date, time.
- Reminders — the evening before.
- Token and queue updates — the patient's token number and a live link.
- Reschedule and cancellation notices.
- Documents — a prescription or bill link after the visit.
- Two-way support — replies to patient questions during the 24-hour window, by staff or a chatbot.
Hospitals should keep clinical content out of routine messages. See ready-to-copy reminder messages.
Is the WhatsApp Business API free?
No. The pricing model works like this (check Meta's current rate card for India, which changes):
- Messages a business starts are sent as pre-approved templates in categories such as utility (for example an appointment confirmation or reminder), marketing (offers) and authentication (OTPs). Meta charges per message, and the rate depends on the category and country.
- Replies to a patient within 24 hours of their last message are service messages, which are not charged in the same way.
- The provider or software that connects you may add its own subscription or per-message fee.
- A free provider plan does not mean free messages. Ask who pays the Meta charges.
Compare the whole bill: platform fee, Meta message charges, setup and support.
How to get the WhatsApp Business API
- Create a Meta business account and verify the hospital's business details.
- Choose a phone number to register for the platform. A number already in use on the Business app must be moved, so decide whether it is your main patient number.
- Pick a route: a Business Solution Provider, a software product that already includes it, or a direct integration with Meta's Cloud API by a developer.
- Create and submit message templates for Meta's approval — for example a booking confirmation with variables for name, doctor and date.
- Get patient opt-in and record it.
- Test with your own number, then switch on for patients.
Most hospitals skip steps 1 to 4 by using hospital software that has it built in.
Skip the setup: OPDly already sends its patient messages through the official WhatsApp Business Platform, so a hospital does not apply for an API account or write templates. See WhatsApp for hospitals.
Rules for healthcare messages
- Opt-in is required. Patients must agree to receive messages from you. Ask at registration and keep a record.
- Templates for first contact. Any message you start must use an approved template. Free-form replies are allowed within 24 hours of the patient's last message.
- Keep sensitive details out. Send a date and a doctor's name, not a diagnosis or a test result in a message. India's Digital Personal Data Protection Act, 2023 expects you to use personal data only for the purpose it was given and keep it secure.
- Do not mix marketing into care messages. Offers sent in a patient's appointment thread teach them to ignore or block you, and marketing templates are charged differently.
- Honour "stop". Stop messaging a patient who asks.
- Do not use unofficial tools that automate a normal WhatsApp account or WhatsApp Web. Meta can restrict or ban the number.
- Never let automation give medical advice.
Common mistakes
- Buying an expensive chatbot platform when only reminders were needed.
- Registering the main patient number without a plan for the existing chats.
- Template rejections from missing variables or promotional wording.
- No record of opt-in.
- Forgetting the 24-hour window, so a staff reply fails.
- Not checking who pays Meta's message charges.
How OPDly uses the WhatsApp Business Platform
OPDly sends its WhatsApp messages through Meta's official WhatsApp Business Platform, so a hospital does not need to apply for an API account or write templates:
- Messages sent: booking confirmation (a token for a walk-in, the date and time for a future booking), a check-in message with the token and a live queue link, an evening-before reminder and a reschedule notice.
- Credits: every hospital gets 300 free WhatsApp credits, extra credits cost 20 paise each, and a message that fails uses no credit.
- Switches: the owner can turn each patient message on or off in Settings → Update Profile → Notifications → WhatsApp to patients.
- Patient consent: ask for it at registration as part of your normal process.
OPDly does not run a chat bot that answers incoming patient messages. See WhatsApp for hospitals, compare clinic WhatsApp bots vs automation, or start a free 30-day trial.
Frequently asked questions
What is the WhatsApp Business API?
Meta's official platform for software to send and receive WhatsApp messages automatically. It is now called the WhatsApp Business Platform and supports approved templates, two-way chats and integrations with systems such as clinic software.
Is the WhatsApp Business API free?
No. Meta charges for business-started template messages, and the provider you use may charge a subscription or margin. Replies within 24 hours of a patient's last message are service conversations, which are not charged the same way. Check Meta's current rate card.
What is the difference between WhatsApp Business and the WhatsApp Business API?
The WhatsApp Business app is a free app for sending messages by hand. The API (platform) connects your software to WhatsApp so messages go out automatically, with approved templates, chatbots and integrations.
How can a hospital get the WhatsApp Business API?
Verify the hospital with Meta, register a phone number, then connect through a provider, a software product that includes it or a developer using Meta's Cloud API. Submit message templates for approval and collect patient opt-in.
Can a hospital send reminders with the WhatsApp Business API?
Yes. Appointment confirmations and reminders are sent as approved utility templates to patients who have opted in.
Do patients have to agree to receive WhatsApp messages?
Yes. WhatsApp's business policy requires opt-in. Ask for it at registration and record it.
Can I use my current clinic number for the API?
Yes, but the number has to be moved from the Business app to the platform, and chats on the app do not carry over the same way. Many hospitals use a separate number for automation.
