What should a doctor's prescription pad have?

A prescription pad saves you writing your details on every sheet, and a chemist will look for them before dispensing. A complete pad in India has:

  • Your name, qualifications and registration number with the name of the medical council (for example "Reg. No. PMC 12345"). The Indian Medical Council ethics regulations ask doctors to show the registration number on prescriptions, certificates and receipts.
  • Clinic name, address and phone, so the patient and the chemist can reach you.
  • OPD timings and the closed day, which saves your reception many calls.
  • Lines for the patient's name, age, sex and date, and if you like, weight and BP.
  • A large blank Rx area with the ℞ symbol, and space for your signature and stamp.

Keep the header to the top quarter of the page, without photos or advertising. For the full format of the prescription itself — medicine names, dose, timing and duration — read our doctor prescription format guide.

A5 or A4?

Most Indian clinics use A5 (half an A4 sheet): it is quick to write on, cheap to print and fits in the patient's file. Choose A4 if you write long notes, investigations or diet advice. To print A5 pads at a local print shop, ask them to print two per A4 sheet and cut.

Letterhead, simple or pre-printed — which style?

Letterhead design gives a coloured, professional header and footer from just your details — good if you are printing new pads. Simple is plain black text that prints sharply on any home printer or photocopier. For pre-printed paper leaves a blank gap at the top, so you can keep using the pads you already paid for and print only the patient lines and Rx area on them.

How to print it

  1. Click Download PDF / Print.
  2. In the print window, pick Save as PDF (or your printer).
  3. Set the paper size to the one you chose, scale to 100% or "Actual size", and turn off Headers and footers.
  4. Use 80–100 GSM paper for pads that don't tear, or take the PDF to a print shop.

No. Everything stays in your own browser — your details are kept on this device so you don't have to type them again, and nothing is sent to OPDly.

Your name, qualifications and registration number with the name of the medical council, plus the clinic's address and phone. The Indian Medical Council ethics regulations ask doctors to display their registration number on prescriptions, certificates and receipts.

Yes. Use the 'Name in local language' field to add a line like 'डॉ. अंजलि शर्मा' under the clinic name. It prints in any Indian script your browser supports.

In the print window choose 'Save as PDF' or your printer, set the paper size to the one you picked (A5 or A4), set scale to 100% or 'Actual size', and turn off 'Headers and footers'.

Yes, with OPDly. The doctor picks medicines from a list and OPDly prints the full prescription on this kind of letterhead — or on your own pre-printed pad — in English or nine Indian languages, and sends it to the patient on WhatsApp.
