Best Hospital Management Software in India for Small Hospitals (2026)
The best hospital management software for small Indian hospitals: HMS vs clinic software, published prices, IPD or OPD, and what to ask before you sign.
By OPDly Team · Updated 23 Oct 2026 · 10 min read
Quick answer: the best hospital management software for a small Indian hospital depends on one question first: how much of your work is in-patient (IPD)? If admissions, wards and discharge summaries are a big part of your day, you need a hospital system with IPD — MocDoc, Healthray, DocPulse and Doctors App's hospital plans all offer it. If your hospital is mostly OPD, with a pharmacy and lab, cloud software built for the front desk does the job for a fraction of the cost — OPDly costs ₹5,988–₹14,388 a year for up to 10 doctors. Published prices for small-hospital systems start at roughly ₹54,000 a year for IPD software, and many vendors share prices only after a demo.
This guide explains what a small hospital actually needs, compares the systems Indian hospitals shortlist most often with their published prices, and lists the questions to ask before you sign.
What "small hospital" software has to do
A 2-to-10-doctor hospital or nursing home in India usually needs these, in this order:
- Registration with one patient ID (UHID) — every visit, test, prescription and bill under one record.
- Appointments and a token queue for each doctor, so reception can run several OPDs from one screen.
- Consultation and prescriptions — typed by the doctor, or written on paper while reception does the typing.
- Billing — consultation, procedures, pharmacy and lab, with cash, UPI and card, dues and refunds.
- Pharmacy — stock by batch and expiry, purchases and the sale counter.
- Lab — test orders from the doctor, results and a printed or WhatsApp report.
- IPD — admissions, beds and wards, nursing notes and discharge summaries — only if you admit patients regularly.
- Staff roles, an audit log and owner reports, so the owner sees every rupee without standing at the counter.
Items 1–6 and 8 decide whether the software works on a busy morning. Item 7 decides which kind of software you can consider at all.
Hospital software vs clinic software: which do you need?
| If your hospital… | You need |
|---|---|
| Admits patients most days, has wards, an OT or ICU, insurance/TPA billing | A full hospital system with IPD |
| Is mostly OPD, with a pharmacy and lab, and admits only occasionally | OPD-focused hospital software — keep a simple register for the occasional admission |
| Has one or two doctors and no pharmacy or lab | Clinic software — see the best clinic management software in India |
Buying a full IPD system for a hospital that is 90% OPD is the most common expensive mistake: longer setup, more training, and a reception screen built around wards instead of the queue.
How much hospital management software costs in India
| Kind | Typical published price | What you get |
|---|---|---|
| OPD-focused cloud software | Roughly ₹6,000–₹15,000 a year | OPD, billing, pharmacy, lab, several doctors |
| Cloud hospital system with IPD | From about ₹54,000 a year; often quoted on request | OPD + IPD, beds, discharge, pharmacy and lab (sometimes as add-ons) |
| Large or corporate HMS/HIS | Quoted after a demo; setup fees are common | Multi-department, insurance, multiple branches |
| Open-source (for example Bahmni) | No licence fee, but you pay for a server, setup and IT support | Full EMR and hospital system you host yourself |

OPDly
Cloud software for the OPD side of a hospital: one patient ID, every doctor's token queue from one reception screen, consultations and prescriptions, one combined bill for consultation, lab and pharmacy, pharmacy stock by batch and expiry, lab orders and reports, staff roles with permissions, an audit log, and a 9 PM WhatsApp summary of the day's collection for the owner. Patients get WhatsApp confirmations, reminders and a live queue link. IPD admissions, beds and wards are not available yet — they are planned — so a hospital with daily admissions would need to manage those separately for now. Plans: Starter ₹5,988, Standard ₹9,588, Pro ₹14,388 a year, plus GST; no setup fee. Compared in detail: OPDly vs MocDoc, OPDly vs DocPulse.
Healthray
One of the few hospital systems with a public price list. Its Hospital Management System Basic plan is listed at ₹54,000 a year per care center (₹1,299 per user a month, billed annually), with OPD, IPD with bed management, lab and inventory in its feature table. Prices exclude taxes and additional providers, and are subject to annual revision. Enterprise plans are quoted on request.
MocDoc
A long-running hospital suite with appointments, billing and insurance, EMR, multi-location management and reporting, plus separate products for pharmacy (PMS) and lab (LIMS) and lab-machine integration. Pricing is not published; it offers a free demo.
DocPulse
A modular hospital management system with IPD, IVR phone booking, telemedicine and ABDM support. Pricing is quoted after a demo, with a one-week assisted trial.
Doctors App
Publishes clinic plans at ₹10,999 and ₹16,999 a year (plus 18% GST), with lab and pharmacy as add-ons. Its hospital plans — IPD registration, ward and bed allocation, IPD billing, discharge summaries — are priced on request.
Bahmni
An open-source EMR and hospital system managed by the Bahmni Coalition, combining registration, clinical services, lab (OpenELIS), inpatient management, stock and billing (Odoo). It is built to be hosted at the hospital itself, so you need a server, an implementation partner or IT team, and someone to maintain it.
Questions to ask before you sign
- Is the price per hospital or per user? A per-user price multiplies with every receptionist, nurse and pharmacist.
- Are pharmacy, lab and IPD included, or add-ons?
- What does a WhatsApp message cost, and is there a free quota?
- Is there a setup fee, a yearly maintenance charge or a price increase at renewal?
- How long does setup take, and who trains your staff?
- Can reception book a walk-in in under a minute? Try it yourself on a busy-day scenario.
- Can you export all your data — patients, visits, bills, stock — if you leave?
- Who answers the phone during your OPD hours?
Red flags
- No price until after a demo for a small hospital. You cannot compare fairly or plan renewal.
- A server to buy and maintain when you have no IT person.
- Setup measured in months for a hospital that only needs OPD, billing, pharmacy and lab.
- Separate logins and separate patient lists for pharmacy and lab — that means retyping, not one system.
Where OPDly fits
OPDly is built for the 2-to-10-doctor hospital where a few people at reception, the pharmacy and the lab keep everything running. It is ready as soon as your account is approved, runs in a browser on the computers and phones you already have, and new staff learn it in minutes. The doctor can keep writing on paper while reception does the typing. If IPD is the centre of your work, choose a system with IPD today; if your hospital is mostly OPD, OPDly covers it at a small-clinic price. Frequently asked questions
Which is the best hospital management software in India for a small hospital?
It depends on how much IPD you do. For hospitals with daily admissions, compare systems with IPD such as Healthray, MocDoc, DocPulse and Doctors App's hospital plans. For OPD-heavy hospitals with a pharmacy and lab, OPD-focused software such as OPDly costs far less and is quicker to set up. Book real patients on a free trial before deciding.
How much does hospital management software cost in India?
OPD-focused cloud software costs roughly ₹6,000–₹15,000 a year. Published prices for cloud hospital systems with IPD start at about ₹54,000 a year, and many vendors quote only after a demo. Open-source systems have no licence fee but need a server and IT support.
What is the difference between HMS, HIS and EMR?
An EMR keeps clinical records — history, vitals, prescriptions, reports. A hospital management system (HMS), also called a hospital information system (HIS), runs the whole hospital: registration, appointments, billing, pharmacy, lab, staff and reports, and usually IPD. Many products combine both.
Is there free hospital management software?
Open-source systems such as Bahmni have no licence fee, but you host them yourself, so you pay for the server, setup and IT support. Most cloud products offer a free trial instead of a free plan.
Does OPDly have IPD?
Not yet. OPDly runs the OPD side of a hospital — appointments, queue, consultation, billing, pharmacy and lab. IPD admissions, beds and wards are planned.
Can hospital software work on a mobile phone?
Cloud software runs in the phone's browser, so the owner or doctor can check the queue and the day's collection from anywhere. Software installed on a hospital server usually works only inside the hospital network.
