Quick answer: under the Clinical Establishment Act in Gujarat — the Gujarat Clinical Establishments (Registration and Regulation) Act, 2021, in force since 13 September 2022 — every clinic, polyclinic, consulting room, dental clinic, lab and hospital in Gujarat must register. You apply online at clinicalestablishment.gipl.in, get provisional registration first and permanent registration later. For a clinic or consulting room the fee is ₹1,000 (provisional) and ₹2,000 (permanent), and it doubles for a late application. Since the March 2026 amendment, the last date is no longer written in the Act — the state government notifies it in the Gazette — so check the portal for the current date before you plan around it.

This guide covers who must register, the full fee table, the documents the form asks for, and the records Gujarat expects you to keep once you are registered.

What changed in 2026

  • The Act originally fixed a deadline for existing establishments, last extended to 30 April 2026.
  • On 25 March 2026 the Gujarat Assembly passed an amendment that removes the fixed date. The government now sets the registration deadline "from time to time" by notification in the Official Gazette, and provisional registration continues until the date the government notifies.
  • By 20 March 2026 Gujarat had issued about 41,000 provisional and about 2,000 permanent registrations, so most clinics are still on provisional certificates.

Because the date now moves by notification, do not rely on a date you heard from a colleague or read in an old article. Look for the latest notice on the portal or ask your Chief District Health Officer (CDHO) or Municipal Health Officer.

Who must register under the Gujarat Act

The Gujarat rules define each type of establishment:

  • Consulting room — where a doctor examines patients, gives advice and writes prescriptions.
  • Clinic — outpatient examination, consultation and prescriptions, including dispensing medicines, by one doctor. Minor procedures such as dressings and injections are allowed.
  • Polyclinic — the same, with more than one doctor.
  • Dental clinic and dental hospital.
  • Hospital — any place that admits in-patients, including nursing homes and maternity homes.
  • Laboratory — pathology, biochemistry, microbiology, genetic or any diagnostic tests.

Two rules catch people out:

  1. One application per system of medicine. If you practise allopathy and homoeopathy from the same premises, you apply separately for each.
  2. A lab inside your clinic does not register separately. A standalone lab or diagnostic centre does.

Establishments run by the Armed Forces are outside the Act.

How to apply for clinical establishment registration in Gujarat

  1. Go to clinicalestablishment.gipl.in and read the guidance document linked on the home page before you start.
  2. Register an account, then log in.
  3. Fill in the application (Annexure-1A): establishment details, owner, the doctor in charge with their GMC or council registration number, type and category, area, OPD rooms and beds, staff, equipment and biomedical waste arrangement.
  4. Upload the documents that apply to you (list below) and pay the fee online.
  5. Download and keep the acknowledgement and, once issued, display the certificate where every patient can see it.

Documents to keep ready

The application asks you to upload, where they apply to your establishment:

  • Fire safety NOC or certificate
  • Biomedical waste authorisation from the Gujarat Pollution Control Board (GPCB)
  • AERB authorisation if you have X-ray
  • PC-PNDT registration if you do ultrasound
  • Drug licence if you run a pharmacy
  • ART or Surrogacy Act registration, blood bank licence — only if relevant
  • 5 to 10 photographs of the establishment from different angles

You will also fill in the qualification and council registration number of each doctor, nurse, paramedic and pharmacist, and list any equipment costing more than ₹10,000.

Minimum standards for a clinic

Annexure 9 of the rules sets the norms an inspector checks. For an allopathic clinic:

  • Space to keep a patient under observation — for an allergic reaction or an IV drip — and a separate waiting area.
  • Staff: at least one registered medical practitioner and one qualified paramedical staff member.
  • Dispensing only by a qualified pharmacist, or by the doctor or under their direct supervision.
  • Biomedical waste segregated and disposed of as per GPCB norms.
  • Prescription slip signed by the doctor with the date and their council registration number.

A polyclinic also needs separate cubicles for doctors seeing patients at the same time, drinking water and a toilet, and the names of the attending doctors displayed in the waiting room.

After registration: the records Gujarat expects

Rule 14 and 15 of the Gujarat rules list your ongoing duties. For an outpatient clinic, the ones that matter every day are:

  • Display your registration certificate and your rates.
  • Give every patient a record of the visit with their name, age, gender, date, diagnosis (provisional or final), the treatment advised and any test results.
  • Keep clinical records, preferably electronically. OPD records must be kept electronically for at least two years; IPD records for at least five years; medico-legal and court cases until the case is closed.
  • Keep an OPD register with, for each patient: serial number, name and address, mobile number, age, gender and provisional diagnosis (Annexure 10, Part D).
  • Report every month to the CDHO or Municipal Health Officer the patients first diagnosed with TB or another notifiable disease at your clinic, with their contact details.
  • Keep staff and equipment records electronically and report changes half-yearly.

Penalties

As reported when the 2026 amendment was passed, an establishment that does not register in time can be fined from ₹10,000 up to ₹5 lakh, and registration can be cancelled for violations. An appeal against the District Authority's decision goes to the State Council within three months.

How OPDly helps

Gujarat asks clinics to keep OPD records electronically — that is what OPDly does as you work. Reception registers each patient once and they get a permanent patient ID. The doctor records vitals, diagnosis and the prescription on each visit, and the prescription prints on your letterhead with the doctor's registration number. Every visit stays searchable for as long as you need it, the day's appointments download to Excel with each patient's name, age, gender and phone, and the patient can get the prescription on WhatsApp. It runs in a browser on any phone, tablet or PC.
